The following terms of use (as may be amended or modified, these "Terms of Use") apply to your use of the Studies.Enroll-HD.org website and all its sub-domains (the "Site"), including all databases accessible through the Site and all materials, services and other content available through the Site and all emails and other communications generated by the Site (collectively, the "Enroll-HD Study Portal") .

 
You acknowledge that the Enroll-HD Study Portal is being made available by CHDI Foundation, Inc. ("CHDI") (referred to herein as "we" or "us") for the sole purpose of conducting the observational study funded by CHDI entitled "ENROLL-HD: A Prospective Registry Study in a Global Huntington's Disease Cohort" (the "Enroll-HD Study").

By accessing, browsing or otherwise using the Enroll-HD Study Portal, you accept these Terms of Use. We reserve the right to modify these Terms of Use at any time and such modifications shall be effective immediately upon the posting of the modified version of these Terms of Use on the Site. Your accessing, browsing or otherwise using the Enroll-HD Study Portal after such modified version of the Terms of Use is posted on the Site, constitutes your acceptance of, and agreement to be bound by, the Terms of Use, as modified. If you do not agree with these Terms of Use, please do not access, browse or use the Enroll-HD Study Portal.

Only "Authorized Users" may access, browse and otherwise use the Enroll-HD Study Portal. For purposes of these Terms of Use, an "Authorized User" means an individual who has been issued a unique user name and password to access, browse and use the Enroll-HD Study Portal by CHDI.

You agree that your user name and password may only be used by you and may not be shared with or used by any other person, including other Authorized Users. You are responsible for protecting the security of your user name and password. You will promptly notify us at if you become aware of any unauthorized use of your user name and/or password, including by other Authorized Users, so that your password can be reset, and you agree to take such reasonable steps as are necessary to prevent any reoccurrence of such unauthorized use.

You will promptly notify us at so that we may deactivate your user name and password if you are no longer working on the Enroll-HD Study or you otherwise wish to terminate your access to the Enroll-HD Study Portal.

 
Use of the Enroll-HD Study Portal

The Enroll-HD Study Portal is being made available to you solely for the purpose of your conduct of activities related to the conduct of the Enroll-HD Study under an agreement (an, "Enroll-HD Study Agreement") between you and CHDI or between the organization for which you are working and CHDI (or another organization which has, directly or indirectly, entered into an agreement with CHDI). You are hereby granted a non-exclusive, world-wide, non-assignable, non-sublicensable, fully paid-up, royalty free, revocable license to access, browse and use the Enroll-HD Study Portal for the sole purpose of conducting activities related to the conduct of the Enroll-HD Study subject to, and in accordance with, the terms and conditions of the Enroll-HD Study Agreement which applies to your conduct of such activities. You agree that any conflict between these Terms of Use and the terms of any Enroll-HD Study Agreement which applies to your conduct of your activities related to the conduct of the Enroll-HD Study to the extent related to your use of the Enroll-HD Study Portal shall be resolved in favor of these Terms of Use.

You may not attempt to gain unauthorized access to the Enroll-HD Study Portal, through hacking, password mining or any other means to circumvent the Enroll-HD Study Portal's security procedures. You may not use the Enroll-HD Study Portal in any manner that could damage, disable, overburden or impair the Enroll-HD Study Portal (including any databases accessible through the Site and all materials, services and other content made available or provided through the Site), or interfere with any other Authorized User's use of the Enroll-HD Study Portal (including any databases accessible through the Site and all materials, services and other content made available or provided through the Site). You may not make use of the Enroll-HD Study Portal (including any databases accessible through the Site and all materials, services and other content made available or provided through the Site) to forge e-mail headers or send bulk unsolicited e-mail messages. 

You may not use, copy adapt, translate, modify, reverse engineer, decompile, disassemble, sub-license, sell, distribute, forward, publish, broadcast, transmit, display, perform or otherwise disseminate any portion of the Enroll-HD Study Portal, except as may be expressly provided in these Terms of Use or any Enroll-HD Study Agreement which applies to your conduct of your activities related to the conduct of the Enroll-HD Study. You may not use any portion of the Enroll-HD Study Portal to create a derivative work. You may not use "web crawlers" or other types of software or hardware technology to automatically download or index any portion of the Enroll-HD Study Portal.

 
You agree that all information made available to you through the Enroll-HD Study Portal, including all databases accessible through the Site or any materials, services or other content made available or provided through the Site, is our confidential information and will be treated by you subject to, and in accordance with, the terms and conditions of the Enroll-HD Study Agreement which applies to your conduct of activities related to the conduct of the Enroll-HD Study.

All right, title and interest (including all copyright and other intellectual property rights) in the Enroll-HD Study Portal, including all databases accessible through the Site or any materials, services or other content made available or provided through the Site, remains with us, our affiliates and third party licensors or any companies or other organizations related thereto. You acknowledge that the databases accessible through the Site and any materials, services or other content made available or provided through the Site may be owned by third parties and provided to CHDI for use under license. You further acknowledge and agree that, except as expressly provided herein, nothing herein grants or constitutes an agreement or obligation to grant you any right, title or interest in and to any copyright or other intellectual property rights in the Enroll-HD Study Portal (including any databases accessible through the Site or any materials, services or other content made available or provided through the Site).

Your accessing, browsing and using the Enroll-HD Study Portal, including all databases accessible through the Site or any materials, services or other content made available or provided through the Site, is also subject to the terms, conditions and notices set forth here - Extended Terms of Use. You may not remove or obscure any copyright or other intellectual property notices contained on or made available through the Enroll-HD Study Portal (including any databases accessible through the Site or any materials, services or other content made available or provided through the Site).

These Terms of Use apply only to the Enroll-HD Study Portal. We may provide links to other websites, but unless otherwise expressly indicated, these other websites have been independently developed by third parties and we do not control what those websites say or do. We are not responsible for other websites' content, information collection practices or use of any information they collect. Links to other websites do not constitute an endorsement by us of those websites or their content, owners or posters.

WE AND OUR AFFILIATES AND AGENTS MAKE NO REPRESENTATIONS OR WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, NON-INFRINGEMENT OR OTHERWISE, AS TO THE OPERATION OF THE ENROLL-HD STUDY PORTAL, OR THE ACCURACY, TIMELINESS OR COMPLETENESS OF DATABASES, MATERIALS, SERVICES AND OTHER CONTENT MADE AVAILABLE ON OR THROUGH THE SITE. YOU EXPRESSLY AGREE THAT YOUR USE OF THE ENROLL-HD STUDY PORTAL AND ANY DATABASES, MATERIALS, SERVICES OR OTHER CONTENT MADE AVAILABLE ON OR THROUGH THE SITE IS AT YOUR SOLE RISK.

Browser Information Collected on the Site

We log IP addresses which are the locations of computers or networks on the Internet, and analyze them in order to improve the utility of the Enroll-HD Study Portal. We also collect aggregate numbers of page hits in order to track the popularity of certain pages and improve the utility of the Enroll-HD Study Portal. Furthermore, we log information about which web browser you use in order to improve the utility of the Enroll-HD Study Portal. We do not gather, request, record, require, collect or track any users' personal information through these processes.
